Pioneers Medical Center Celebrates November as National Home Care Month
Meeker, CO — More than 11 million Americans receive home-delivered health care from home health care providers.
Pioneers Home Health Care nurses, nursing assistants, and therapists make approximately 2,600 home care visits each year. In their honor, Pioneers Medical
Center joins the National Association for Home Care & Hospice in celebrating November as National Home Care Month under the theme, “
Compassionate Health Care Delivered to Your Doorstep.”

Through technological advances, home-delivered health care has grown far beyond basic professional nursing and home care aide services.
Pioneer’s home care agency offers a wealth of nursing, physical, occupational, respiratory and speech therapies, podiatry, medical equipment and
home medical supplies, and personal care, including 24-hour Lifeline service.
Pioneers Home Care has been providing home health services for16 years. It provides a broad range of in-home health and supportive services
for eastern Rio Blanco County’s disabled and chronically ill patients and their families. Its care giving team offers services ranging from basic assistance
with daily living to advanced medical treatment.
Pioneers Hospital Home Health team works with you and your physician to meet your needs during an illness or while you are recovering from an
accident or surgery. Home health can be an alternative to the nursing home or assisted living center. Pioneer’s Home Health team provides the following
services:
Skilled Nursing — experienced Registered Nurses provide medication and pain management, IV therapy, wound care and catheter/ostomy management.
Certified Nurse Assistant — assist with personal hygiene, bathing, ambulation, exercises, skin and wound care.
Occupational Therapy — therapists improve independent daily living skills through environmental adaptation, education and exercise.
Physical Therapy — provides exercise to improve strength, coordination, and balance; assists with pain control and wound care management. As well as,
teaching safe ambulation in the home.
Lifeline Emergency Response System— can be installed in a home to allow direct access to the 24-hour on-call response center.
Medication Management—includes RN review of prescribed medications on a bi-monthly basis.
Foot Care Clinics are offered on a monthly basis.
You may request Pioneers Home Health Services through your physician or call the Home Health office (878-9265) and a nurse
will coordinate a plan with your physician.
Home care services can be paid for by a variety of insurance providers, including Medicare and Medicaid, or directly by patients and their families.
Despite massive cutbacks in government funding, home care continues to be the preferred means of receiving quality health care services for millions of Americans,
allowing them to maintain their independence.
