A confidential record of your diagnosis, treatment, and progress is maintained in our
Medical Records department. This information is a vitally important aide for you, and
allows your provider a chronological record of your care. Medical records are kept in
strict confidence and in accordance with Medical Staff Bylaws, Rules, and Regulations,
as well as standards set forth by the Colorado State Department of Health.
You have the right to review and obtain copies of your medical record. Requests for
these records are handled in accordance with Colorado State Law and the policies and
procedures of Pioneers Hospital. Any patient desiring access to their medical record may
contact the department for assistance. The Medical Records department is open from
8:00a.m. to 4:30p.m. Monday through Friday, and can be reached at (970) 878-9267.
