Home Health Services

Home Health visits patients Home Health delivers patient care to qualifying persons in their homes. A physician's referral is necessary before a patient can be evaluated for these services. You may request our services from your physician and ask whether you qualify. Assistance includes skilled nursing, certified nurse aide for personal care and home assistance, and occupational and physical therapy. Home Health services are Medicare and Medicaid-eligible under certain criteria.

Our agency also offers "Private Pay" Personal Care services. We offer a telephone Lifeline Program that can be installed in homes to allow direct access to the 24 hour on-call response center for emergency assistance or for security reasons. The Lifeline Program is not reimbursed by Medicare, but clients may qualify for resource assistance. Individual payment plans can also be established.
